1) Click on the Emergency Contact button below to download the from.

2) Open the file using Acrobat Reader DC. You may download it for free by clicking the "Acrobat Reader DC" button below.

3) Click on the fields and start typing.

​

4) For the signatures, click on the SIGN PEN icon.

​

5) Click on "Add Signature."

​

6) Click on the "Draw" icon.

​

7) Sign your name on the popup signature window. You can use the trackpad on your laptop, the mouse from your desktop PC or your finger/stylus on your smartphone. Be sure to click on ALL the signature lines.

8) For the Photo release section, use the "Add Initial" option.

​

9) Save your document and email it back to us  (kidskoracademy@gmail.com) with the email subject "Emergency Contact Form".
You may also print it out and return it to Room 30 if you are unable to email it back to us.
